Due to the nature of the sport, there are some medical conditions that must be considered. Click here to download a copy of the SCUBA Medical Statement. This form should be downloaded and filled out prior to your arrival here by anyone who is planning on SCUBA diving. The reason for this is that anyone who answers yes to any of the questions on the medical statement MUST get their primary care physician to complete the medical form otherwise they will not be able to dive.
